In this episode, Dr. South discusses the use of C02 laser therapy for the treatment of vulvovaginal atrophy. Atrophy is the thinning of the walls of the vulva, vagina, bladder, and urethra due to the decreasing levels of estrogen with menopause. Genitourinary syndrome of menopause, or GSM, is an all-encompassing term used to describe all the symptoms associated with atrophy. During this podcast, Dr. South discusses the symptoms, diagnosis, and specifically the use of CO2 laser therapy for the treatment of this condition.
In your body, certain chemicals travel from nerve cells to muscle cells to make your bladder contract so that you can urinate. For people who have OAB, these muscles contract uncontrollably, creating leakage, the sudden strong need to go, and going too often. BOTOX® is placed directly in the bladder muscle, helping block the nerve signals that trigger OAB. In this episode, Dr. South discusses the use of BOTOX® for the treatment of OAB. You will hear her patients talk about their experience with this treatment option.
In this episode of Girl Problems, Dr. South discusses a form of bladder leakage called stress urinary incontinence. Female stress incontinence is very common and can affect women of all ages. The symptoms include uncontrolled bladder leakage with cough, laugh, sneeze, and other activities that create pressure on the bladder. You will learn about how stress incontinence is diagnosed, hear real women talk about their symptoms, and learn about treatment options.
Urinary tract infections affect women of all ages and all stages of life. We see UTIs in young, healthy women both sexually active and not and in older, postmenopausal women. The evaluation and treatment of UTI cost more than 2 billion dollars a year in the United States. Women who experience recurrent UTI find this condition very distressing. Dr. South discusses the diagnosis and treatment of recurrent urinary tract in this episode of Girl Problems.
Overactive Bladder is a combination of symptoms, including urinary frequency, urgency, and urinary urge incontinence. Bladder function involves the coordination of the brain and muscles through our nerve signals. With an overactive bladder, regardless of what our brain wants our bladder to do, our bladder has a mind of its own and contracts at inopportune times. Medtronic Bladder Control Therapy delivered by the InterStim™ systems uses gentle nerve stimulation to correct the bladder-brain communication pathway and restore bladder function. This therapy provides relief and restores bladder function by gently stimulating the sacral nerves. Urogynecologists are Obstetrician Gynecologists who have spent several years of additional training to learn how to care for specific conditions that affect the pelvic floor. Urogynecologists can treat certain conditions medically but they are also surgeons and often use minimally invasive surgical techniques to treat pelvic organ prolapse and urinary incontinence.
Mary South is a native of Northeastern Ohio. After growing up in Wooster, Ohio she graduated from the College of Wooster and earned her medical degree from Case Western Reserve University School of Medicine. She and her husband then moved to North Carolina where she completed a four year residency program in Obstetrics and Gynecology at the University of North Carolina at Chapel Hill, and then a three year fellowship in Urogynecology at Duke University Hospital.

Bulkamid is a soft, smooth hydrogel that consists of 97.5% water and 2.5% polyacrylamide.
Once injected, Bulkamid provides additional volume to the urethra and acts as a scaffold for cells to grow through helping to provide long lasting relief of stress urinary incontinence symptoms.
Fecal incontinence often called accidental bowel leakage, is the inability to control bowel movements, resulting in leakage of solid or liquefied stool. Symptoms of this chronic condition – which affects about 6% of women under age 40, and 15% of older women in the United States – ranges from moderate to socially debilitating. While men also suffer from fecal incontinence, it’s more far common among women due to their reproductive anatomy and hormonal changes during menopause.

Many women suffer from pelvic pain and don’t understand why they hurt and how to cure their pain.
The most common causes of pelvic pain include endometriosis, vaginal atrophy (lack of estrogen stimulation to the vaginal tissues), scarring related to previous vaginal surgery and spasm of the pelvic floor muscles.
We offer a careful evaluation of possible causes for your pain and many different treatment options. Schedule a consultation to begin your healing.
Urinary Tract Infections can occur anywhere along the urinary tract including the bladder and kidneys. These infections are usually caused by bacteria found in the colon, such as E. coli. 40% of women will have a urinary tract infection in their lifetime. Those women who have had an urinary tract infection are more likely to have recurrent infections.
Symptoms of an infection usually include burning and pain with urination. In addition, sometimes women feel bladder pressure or a back ache and can sometimes see blood in their urine.
Most infections can be treated with a short course of antibiotics, but if infections recur or persist, sometimes a long-term course of a low-dose suppressive antibiotic or post-coital (after sex) prophylactice antibiotic is required. A urogynecologist or urologist can help you determine what type of treatment regimen is appropriate for you.
As women age and go through menopause, the estrogen levels decline. This results in a thinning of the vaginal tissue or epithelium. We refer to this as vaginal atrophy.
Vaginal atrophy can result in vaginal dryness, burning, discomfort or pain with intercourse, and recurrent vaginal infections or urinary tract infections.
Although vaginal lubricants can help, the mainstay of treatment, until recently, has been topical estrogen cream. Topical estrogen cream or vaginal estrogen tablets are very safe and effective for treating vaginal atrophy. However, many women don’t want the mess of applying the cream regularly or the hassle of remembering to place a vaginal tablet twice per week. In addition, some women can not use vaginal estrogens due to other medical problems or simple don’t want to use hormonal treatments.
We are now offering a new non-hormonal treatment, called Mona Lisa Touch by Cynosure, to relieve symptoms of vaginal atrophy. We are currently the only medical practice in the Akron area with this technology. Please click here to learn more about Mona Lisa Touch therapy for vaginal atrophy.
This is an abnormal connection or passageway between the urinary or gastrointestinal tract and genital structures. They can occur between any of the organs and structures of the pelvic region. A fistula allows urine or stool to continually exit through and out the urogenital tract. The fistula can result most commonly from infection, prolonged labor and delivery during childbirth, hysterectomy, radiation therapy, or inflammation (e.g., diverticulitis).
Temporary treatments like a urinary catheter or a small stent can help control symptoms. In most cases, surgery is the most effective treatment. Evaluation and discussion with your doctor is the first step in understanding your treatment.
Lichen sclerosus is a chronic skin inflammatory disorder that causes white patchy skin that most commonly affects the genital and anal areas. Most postmenopausal women are at higher risk. Women often complain of significant dryness, itching, and pain with intercourse.
Special care to the tissues affected with the condition is important to help control symptoms. Many patients respond favorably to steroid ointment and modifications to hygiene habits. For recalcitrant cases, other options are available. We also offer Mona Lisa Touch laser treatments for patients suffering from lichen sclerosus.
A woman’s labium becomes enlarged over time for several reasons that may include childbirth, injury, genetics, or for no apparent reason. Young girls and young women can also have enlarged or elongated labia. Women with abnormal labia can suffer pain during intercourse, experience discomfort when performing everyday activities, or they may feel extremely self-conscious.
Labiaplasty involves surgically trimming the labial tissue. Special care is taken to remove only enough excess tissue to retain adequate laxity of stretchable tissue to allow for a smaller, but still natural-appearing labia.
Candidates who choose labiaplasty may do so for functional or aesthetic reasons and the procedure is frequently performed to reduce the size of one or both sets of labia.

Medtronic Interstim therapy is an effective long-term treatment option for Overactive Bladder with or without Urge Urinary Incontinence and Bowel Problems including Fecal Incontinence. Interstim is also an available treatment option for patients who have trouble emptying their bladders (aka voiding dysfunction).

Laser Therapy for Vaginal Atrophy
One of the most difficult changes that is associated with menopause is vaginal dryness. The vagina, and the surrounding the vaginal opening including the clitoris and labia minora are all richly supplied with estrogen receptors. When estrogen decreases at menopause, these receptors are left empty and the whole area atrophies. This causes the lining of the vagina to become thinner, less able to produce moisture and more easily injured. Sexual intercourse becomes less comfortable and even painful.

Microscopic hematuria is the finding of blood on a urine dipstick or by seeing red blood cells in the urine under microscopic magnification. Often, women with microscopic hematuria have no symptoms but their doctor finds blood on a routine urinalysis. A patient with microscopic hematuria can not see the blood in their urine with the naked eye. A patient who sees blood in their urine should contact their doctor as soon as possible.
A patient with microscopic hematuria may have something in their history that helps determine why they have blood in their urine. These include a recent or current urinary tract infection, a recent upper respiratory tract infection, a family history of kidney disease, flank pain (possible kidney stones), menstruation, trauma, recent travel, and certain medications.
Oftentimes, we are unable to determine the exact cause for microscopic hematuria.
A dipstick urinalysis is very sensitive for detecting blood in the urine so this can result in a high rate of false-positive tests–meaning although the urinalysis shows blood, if we examine the urine under a microscope we may not see any red blood cells. The first step is to send the urine for microscopic urinalysis and confirm there are at least 3 or more red blood cells seen per high power field. If so, then we proceed with a full evaluation. A full evaluation usually includes blood work to evaluate kidney function, a cystoscopy whereby we use a camera to look inside the bladder, and finally a CT scan of the abdomen and pelvis or a renal (kidney) ultrasound.
Treatment of microscopic hematuria is based on the underlying etiology. If a patient is found to have a urinary tract infection, this can be treated with antibiotics. Kidney stones can be treated or removed if needed. If a tumor or growth is found in the urinary tract, this can be removed. Treatment of any underlying kidney disease can be initiated.
Again, many times a workup does not identify a cause, in which case treatment may not be necessary but the patient may need monitoring or repeat evaluation in the future.
Many women suffer from pelvic pain and don’t understand why they hurt and how to cure their pain. The most common causes of pelvic pain include endometriosis, vaginal atrophy (lack of estrogen stimulation to the vaginal tissues), scarring related to previous vaginal surgery, bladder pain syndromes (i.e. Interstitial Cystitis), and spasm of the pelvic floor muscles. Spasms of the pelvic floor are often a result of pain in the pelvis for other reasons. For example, a woman may have vaginal atrophy which causes discomfort, and as a result, the muscle tone in her pelvis increases, and this chronic tension results in further pain.

Pelvic organ prolapse happens when the muscles and tissues supporting the pelvic organs (the uterus, bladder, or rectum) become weak or loose. As the muscles and tissues weaken, the pelvic organs drop or press into or out of the vagina. Most of the time, pelvic organ prolapse is the result of carrying—and vaginally delivering children, which weakens the pelvic floor.

A urethral diverticulum is a pocket or sac that forms along the urethra. Although the exact cause of urethral diverticula is not known, they are likely due to repeated infections in a periurethral gland that ultimately results in blockage. The blocked gland then likely bursts into the urethra creating the communication between the sac and the urethra. This condition is felt to be acquired rather than congenital (these are rarely if ever seen in babies).
Women with urethral diverticulum may experience postvoid dribbling (drips after urinating), dysuria (burning with urination), and dyspareunia (pain with intercourse). In addition, women may experience, chronic or recurrent urinary tract infections, urinary frequency and/or urgency, hematuria, bloody urethral discharge, urinary incontinence, urinary retention, pelvic or urethral pain, or a vaginal bulge or mass.
Not all urethral diverticula need to be treated. However, if a woman is symptomatic, surgical excision is the treatment of choice. Often the surgeon will order an MRI of the pelvis to identify the complete extent of the diverticulum and to ensure there are no solid components. The surgery itself usually takes less than an hour and is performed on an outpatient basis. Usually, the patient will need to have a foley catheter in place to drain the urine for about 7-10 days after the procedure. This ensures proper healing and prevents breakdown of the sutures. Risks of the procedure include bleeding, infection, recurrent diverticulum formation, urethro-vaginal fistula, and urethral stricture. However, complications from this procedure are usually very low.
Urethral mucosal prolapse occurs when the lining of the urethra protrudes outside the opening of the urethra (urethral meatus). This can occur in young girls, postpartum women (especially if breastfeeding), and postmenopausal women. Urethral prolapse is caused by low levels of estrogen. The tissue loses elasticity and slides down and out of the inside of the urethra. Some people use the word “prolapse” and “caruncle” interchangeably. The only difference between the two is that urethral prolapse is completely prolapsed on all edges and a caruncle is just prolapse on the lower edge of the urethral opening.
Many women with this condition do not have any symptoms. However, sometimes women experience spotting or pain if the prolapse is extreme.
Treatment is not necessary unless the symptoms are bothersome. Topical estrogen cream is the usual first line of therapy. If topical estrogen does not improve symptoms or the patient does not want to use estrogen, CO2 laser treatments with Mona Lisa Touch are highly effective. For this condition, laser therapy is covered by insurance and is not painful. In extreme cases, surgical excision of the prolapsed tissue is sometimes needed.
Do you leak when you cough, laugh or sneeze? You may have stress urinary incontinence. This condition can often be treated with a simple outpatient procedure.
Do you often urinate urgently and can’t get there in time? You might have overactive bladder. Some people have urinary frequency but don’t lose urine on the way to the bathroom. Others can’t hold their bladder and have to wear pads on a daily basis.

Stress Urinary Incontinence (SUI) is the involuntary leakage of urine during activities such as coughing, sneezing, lifting, laughing or exercising. SUI affects at least 10-20% of women, many of whom do not realize that there are simple, effective treatment options available. SUI affects the quality of women’s lives in many ways. Incontinence may limit women’s social and personal relationships, as well as limiting physical activity.

OAB is the name given to the following collection of bladder symptoms:
OAB affects women (and men) of all ages, and is not simply a result of getting older.
OAB symptoms are caused by the bladder muscle squeezing to empty out urine inappropriately. This often happens without warning, and when you do not want it to.
Your doctor or nurse will test your urine to rule out an infection, which is a common cause of OAB symptoms. You may have other tests to look for bladder stones and growths as well. OAB can also be caused by conditions affecting the nervous system. If you have had a previous operation for stress incontinence, you may also be more likely to have OAB. The amount and type of liquids that you drink may also contribute to your symptoms; for example, caffeinated drinks are thought to significantly worsen OAB symptoms.
For many women however, the exact cause of their OAB is never found. Despite this, there are many treatments which can help you to manage your symptoms.
As urine is produced and fills the bladder, the bladder (detrusor) muscle relaxes and stretches to accommodate the fluid. When the bladder is filled to a certain level, an urge to pass urine is felt, and when it is appropriate, the brain signals the detrusor muscle to contract and the urethral sphincter to relax, thus allowing urine to be passed. The bladder usually needs to be emptied about 4-7 times per day, and once or twice at night.
The urethra and bladder are supported by the pelvic floor muscles, which contract during coughing, sneezing and exercise to prevent leakage. Weakness in the muscles or damage to the Bladder Neck support can result in leakage.
A recurrent UTI is officially defined as three episodes in the previous 12 months or two episodes within the previous 6 months. It is generally accepted that recurrent UTIs occur due to either reinfection or persistent infection.

As women age and go through menopause, the estrogen levels decline. This results in a thinning of the vaginal tissue or epithelium. We refer to this as vaginal atrophy.
Vaginal atrophy can result in vaginal dryness, burning, discomfort or pain with intercourse, and recurrent vaginal infections or urinary tract infections.
Although vaginal lubricants (Uberlube!) can help, the mainstay of treatment, until recently, has been topical estrogen cream. Topical estrogen cream or vaginal estrogen tablets are very safe and effective for treating vaginal atrophy. However, many women don’t want the mess of applying the cream regularly or the hassle of remembering to place a vaginal tablet twice per week. In addition, some women can not use vaginal estrogens due to other medical problems or simply don’t want to use hormonal treatments.

Implantation of a mid-urethral sling is the standard surgical treatment for stress urinary incontinence (SUI) in women. Biofeedback for pelvic floor muscle retraining is a treatment to help patients learn to strengthen or relax their pelvic floor muscles in order to improve bowel or bladder function and decrease some types of pelvic floor pain. It is a painless process that uses special sensors and a computer monitor to display information about muscle activity. This information or “feedback” is used to gain sensitivity, and with practice, control over pelvic floor muscle function. An important part of pelvic floor biofeedback therapy is consistent practice of the pelvic floor muscle exercises at home. With biofeedback, an individual can learn to stop using the incorrect muscles and start using the correct ones.
Percutaneous Tibial Nerve Stimuation (PTNS) is a low-risk, non-surgical treatment. PTNS works by indirectly providing electrical stimulation to the nerves responsible for bladder and pelvic floor function. During PTNS treatment, the patient’s foot is comfortably elevated and supported. Also during treatment, a slim needle electrode is placed near the nerve at the ankle known as the tibial nerve. A device known as the Urgent PC Stimulator is connected to the electrode and sends mild electrical pulses to the tibial nerve. These impulses travel to the sacral nerve plexus, the group of nerves at the base of the spine responsible for bladder function.
By stimulating these nerves through gentle electrical impulses (called neuromodulation), bladder activity can be changed. Because this change happens gradually, patients receive a series of 12-weekly, 30-minute treatments. After the 12 treatments, when the patient’s response to therapy is assessed, occasional treatments may be needed to sustain symptom improvement.
PTNS is a form of electroacupuncture.
This therapeutic treatment is for patients experiencing overactive bladder (OAB) symptoms of urinary urgency, urinary frequency and/or urge incontinence. PTNS has been approved by regulatory agencies outside the United States to treat fecal incontinence, but has not been approved for this usage in the US at this time. PTNS is generally used after behavior modifications, Kegel exercises and failure of medications.
There are patients who should not be treated with PTNS. These patients are those with pacemakers or implantable defibrillators; patients prone to excessive bleeding; patients with nerve damage that could impact either tibial nerve or pelvic floor function; and patients who are pregnant or planning to become pregnant during the duration of the treatment.
